To upload the evaluation tools compatible with New Innovations (NI) system requirements, please log on to NI at your institution. In the right upper corner of your screen, click on Help > Contact Us and submit a Support Request by entering the following information:
- Area of Software: Select Evaluations (Questionnaires)
- Summarize your request: Enter Endocrinology Milestone Evaluation Template
- Details: Enter Please add the Endocrinology Milestone Evaluation Template to my program
- Click Submit Request
- There are 12 evaluation tools that will be uploaded to your program. Each question in these evaluation tools has been mapped to the appropriate sub-competencies and will automatically populate the reporting milestones template.
